标题：肠型十二指肠乳头癌的MRI诊断价值

      【摘要】 目的 探讨磁共振成像(MRI)诊断肠型十二指肠乳头癌的临床价值，提高影像诊断能力。方法 回顾性分析2012年1月至2020年12月在东台市人民医院诊治的16例经手术病理证实的肠型十二指肠癌患者的MRI影像资料，所有患者均在MR常规扫描的基础上进行磁共振胰胆管成像(MRCP)薄层扫描。结果 16例患者中13例为突入十二指肠内小结节，MRCP示胆总管下端8例杯口状、5例截断状狭窄，3例为乳头区浸润生长，局部管壁增厚，MRCP示胆总管下端鼠尾状狭窄。本组中4例胆总管轻度扩张，12例中重度扩张，其中8例合并胰管扩张，出现双管征；10例显示胰胆管“苹果把征”。结论 肠型十二指肠癌的常规MRI及MRCP影像具有特征性，是敏感而有效的影像学检查方法。

Diagnostic value of MRI in duodenal papilla carcinoma of intestinal type.

【Abstract】 Objective To investigate the clinical value of magnetic resonance imaging (MRI) in the diagnosisof duodenal papilla carcinoma of intestinal type. Methods Sixteen MRI images of duodenal papilla carcinoma of intes-tinal type confirmed by operation and pathology were retrospectively analyzed. All the cases underwent MR scanningand magnetic resonance cholangiopancreatography (MRCP) thin layer scanning. Results There were 13 cases of nod-ules protruding into the duodenum. MRCP showed 8 cases of cup-shaped, 5 cases of truncated stenosis, and 3 cases of in-filtration patch and intestinal wall thickening in papillary region, with mouse-tail stenosis of the inferior extremity ofcommon bile duct. In this group, there were 4 cases with mild dilatation of common bile duct and 12 cases with moder-ate to severe dilatation. Among them, 8 cases with dilatation of pancreatic duct presented dual ducts, and 10 casesshowed“apple root sign”of pancreatic bile duct. Conclusion MRI and MRCP of duodenal papilla carcinoma of intesti-nal type are characteristic, and they are sensitive and effective imaging methods.
