      标题:2型糖尿病肾病患者血清UA、Cys C、尿RBP水平及其与mAlb的相关性

      【摘要】 目的 检测2型糖尿病肾病患者血清尿酸(UA)、胱抑素C(Cys C)、尿视黄醇结合蛋白(RBP)水平,并探讨其与尿微量白蛋白(mAlb)的相关性。方法 选取2017年3月至2019年3月江门市人民医院收治的200例2型糖尿病肾病患者作为观察组,同期选取健康体检者 200例作为对照组,比较两组受检者血清中UA、Cys C和β2微球蛋白(β2-MG)、尿RBP、尿N-酰-B-D-氨基葡萄糖苷酶(NAG)、mAlb、尿β2-MG和尿清蛋白/尿肌酐(UACR)水平和肾功能常规指标[肌酐(CR)、尿素(BUN)],并采用Pearson相关性分析法分析尿液中mAlb与肾功能各项指标的相关性。结果 观察组患者血清中 CR、BUN、UA、Cys C和β 2-MG水平分别为(105.27±10.37) μmol/L、(6.48±1.12) μmol/L、(473.27±87.64) μmol/L、(2.99±1.31) mg/L、(4.92±1.88) mg/L,明显高于对照组的(63.25±11.45) μmol/L、(5.31±1.03) μmol/L、(301.11±55.88) μmol/L、(0.73±0.12) mg/L、(0.78±0.21) mg/L,差异均有统计学意义(P<0.05);观察组患者尿液中RBP、NAG、mAlb、β2-MG和UACR水平分别为(8.13±1.42) mg/L、(16.46±5.34) U/L、(36.48±5.19) mg/L、(0.28±0.06) mg/L、(62.59±3.89) mg/g,明显高于对照组的(0.51±0.16) mg/L、(7.51±2.45) U/L、(14.32±2.05) mg/L、(0.07±0.02) mg/L、(3.73±0.22) mg/g,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,尿液中mAlb与肾功能各项指标均呈正相关关系(P<0.05)。结论 2型糖尿病肾病患者血清UA、Cys C和β2-MG水平,尿RBP、NAG、mAlb、β2-MG和UACR水平以及CR、BUN均明显升高,mAlb与各项指标均有一定的相关性。

Relationship between serum UA, Cys C, urinary RBP and mAlb in patients with type 2 diabetic nephropathy.FAN Xiu-jing, GU Jie-chao, CHEN Xin-hao.

【Abstract】 Objective To detect the levels of serum uric acid (UA), cystatin C (Cys C) and urinary retinol bind-ing protein (RBP) in patients with type 2 diabetic nephropathy, and to explore the correlation between them and urinarymicroalbumin (mAlb). Methods A total of 200 patients with type 2 diabetic nephropathy, who admitted to JiangmenPeople's Hospital from March 2017 to March 2019, were selected as the observation group. At the same time, 200healthy people were selected as the control group. The serum levels of UA, Cys C, and β2-microglobulin (β2-MG), uri-nary RBP, urine N-acetyl-BD-glucosaminidase (NAG), mAlb, urinary β 2-MG and urinary albumin-creatinine ratio(UACR) and the changes of renal function routine indexes (creatinine [CR], urea [BUN]) were compared between thetwo groups. Pearson correlation analysis was used to analyze the correlation between mAlb in urine and renal function.Results The levels of CR, BUN, UA, Cys C and β2-MG in serum of the observation group were (105.27±10.37) μmol/L,(6.48±1.12) μmol/L, (473.27±87.64) μmol/L, (2.99±1.31) mg/L, (4.92±1.88) mg/L, respectively, which were significant-ly higher than corresponding (63.25±11.45) μmol/L, (5.31±1.03) μmol/L, (301.11±55.88) μmol/L, (0.73±0.12) mg/L,(0.78±0.21) mg/L in the control group (all P<0.05); the levels of RBP, NAG, mAlb, β2-MG and UACR in the observa-tion group were (8.13±1.42) mg/L, (16.46±5.34) U/L, (36.48±5.19) mg/L, (0.28±0.06) mg/L, (62.59±3.89) mg/g, whichwere significantly higher than corresponding (0.51±0.16) mg/L, (7.51±2.45) U/L, (14.32±2.05) mg/L, (0.07±0.02) mg/L,(3.73±0.22) mg/g in the control group (all P<0.05); Pearson correlation analysis showed that there was a positive correla-tion between the urinary mAlb and renal function indexes (P<0.05). Conclusion Serum UA, Cys C and β2-MG levels,urinary RBP, NAG, mAlb, β2-MG and UACR levels, CR and BUN were significantly increased in patients with type 2diabetic nephropathy, and mAlb was correlated with various indicators.
