      标题:高血压患者视网膜病变与血清脂蛋白相关磷脂酶A2的相关性

      【摘要】 目的 检测高血压患者血清脂蛋白相关磷脂酶A2 (Lp-PLA2)水平,并探讨其与视网膜病变的相关性。方法 选取2019年1月至2020年8月在南通大学附属泰州市人民医院心内科住院的116例高血压患者为研究对象,根据是否合并视网膜病变分为单纯高血压64例(对照组)和高血压视网膜病变(HRP) 52例(观察组)。检测并比较两组患者的血清Lp-PLA2、血糖、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白AI (ApoAI)、载脂蛋白B (ApoB)指标。采用Spearson相关性分析及二元Logistic回归分析探讨Lp-PLA2与视网膜病变的相关性。结果 观察组患者的女性比例、高血压病程、糖尿病患病率、Lp-PLA2水平分别为53.85%、(9.58±1.32)年、57.69%、(152.35±39.48) ng/mL,明显高于对照组的34.38%、(7.34±1.12)年、37.5%、(112.18±28.86 ) ng/mL,差异均具有统计学意义(P<0.05);经 Spearson相关分析结果显示,Lp-PLA2与TG、脂蛋白(a)、ApoAI/ApoB、HRP呈正相关性 (r=0.077、0.139、0.051、0.165,P<0.05),与 HDL-C呈显著负相关 (r=-0.084,P<0.05);二元Logistic回归分析结果显示,在校正性别、高血压病程、糖尿病病史后,Lp-PLA2仍是HRP的独立危险因素(P<0.05)。结论 Lp-PLA2与HRP具有相关性,是HRP的独立危险因素,提示早期检测Lp-PLA2水平对预测高血压患者视网膜病变的发生具有一定的临床价值。

【Abstract】 Objective To investigate the lipoprotein associated phospholipase A2 (Lp-PLA2) level in hyperten-sive patients and to explore the relationship between Lp-PLA2 and retinopathy in hypertensive patients. Methods Thesubjects were selected from the Department of Cardiology, Taizhou People's Hospital Affiliated to Nantong Universi-ty from January 2019 to August 2020, who were divided into hypertension without retinopathy group (control group,64 cases) and hypertension with retinopathy group (observation group, 52 cases). Serum Lp-PLA2, blood glucose, tri-glyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholester-ol (LDL-C), ApoAI, ApoB were detected and compared between the two groups.
