      标题:3D-CCI-HyCoSy技术在不孕症患者输卵管病变诊断中的价值

      【摘要】 目的 研究三维编码对比增强子宫输卵管超声造影(3D-CCI-HyCoSy)技术在不孕症患者输卵管病变诊断中的价值。方法 选取2017年10月至2019年10月在西安高新医院接受治疗的120例不孕症患者作为研究对象,分别对其进行3D-CCI-HyCoSy检查及腹腔镜通液术检查,通过腹腔镜通液术检测结果分析不孕症患者输卵管病变情况,采用Kappa检验分析3D-CCI-HyCoSy检查诊断不孕症患者输卵管病变与腹腔镜通液术检测结果的一致性,同时比较两种方式检测后患者不良反应发生情况。结果 经腹腔镜通液术检查后发现输卵管通畅、通而不畅、阻塞发生率分别为 18.33%、41.67%、40.00%,其中通而不畅及阻塞发生位置主要以伞部为主,发生率分别为50.00%、56.25%。3D-CCI-HyCoSy诊断不孕症患者输卵管通而不畅的灵敏度、特异度、准确率分别为 92.00%、95.71%、94.17%,与腹腔镜通液术检测结果的一致性Kappa检验值为0.880;3D-CCI-HyCoSy诊断不孕症患者输卵管阻塞的灵敏度、特异度、准确率分别为93.75%、97.22%、95.83%,与腹腔镜通液术检测结果的一致性Kappa检验值为0.913。3D-CCI-HyCoSy检测方式不良反应总发生率为 20.83%,明显低于腹腔镜通液术的33.33%,差异有统计学意义(P<0.05)。结论 3D-CCI-HyCoSy技术对于诊断不孕症患者输卵管通而不畅和阻塞具有较高价值,且不良反应少,安全性较高。

Value of 3D-CCI-HyCoSy in the diagnosis of tubal diseases in infertile patients.

【Abstract】 Objective To study the value of three-dimensional coded contrast-enhanced hysterosalpingography(3D-CCI-HyCoSy) in the diagnosis of tubal diseases in infertile patients. Methods A total of 120 infertile patients whovisited Xi'an High-tech Hospital for treatment from October 2017 to October 2019 were selected as the research objectsand were examined by 3D-CCI-HyCoSy and laparoscopic hydrotubation respectively. The tubal lesions of infertile pa-tients were analyzed through the results of laparoscopic hydrotubation. Kappa test was used to analyze the consistencybetween 3D-CCI-HyCoSy examination and laparoscopic hydrotherapy in diagnosing tubal diseases of infertile patients,and the incidence of adverse reactions of patients after the two methods were compared. Results After laparoscopicdrainage, it was found that the incidences of unobstructed, partially obstructd, and obstructed fallopian tubes were18.33%, 41.67%, and 40.00%, respectively. The location of partially obstructed and obstructed fallopian tubes was main-ly the umbrella, and the incidences were 50.00% and 56.25%, respectively. The sensitivity, specificity, and accuracy of3D-CCI-HyCoSy in the diagnosis of tubal obstruction in infertile patients were 92.00%, 95.71%, and 94.17%, respective-ly, and the Kappa test value was 0.880. The sensitivity, specificity, and accuracy of 3D-CCI-HyCoSy in diagnosing tubalobstruction of infertile patients were 93.75%, 97.22%, and 95.83%, respectively, and the Kappa test value was 0.913.The total adverse reaction rate of 3D-CCI-HyCoSy was 20.83%, which was significantly lower than 33.33% of laparo-scopic drainage, and the difference was statistically significant (P<0.05). Conclusion 3D-CCI-HyCoSy is of high val-ue in the diagnosis of tubal obstruction and obstruction in infertile patients, with less adverse reactions and high safety.
