      标题:右美托咪定辅助超声引导股神经-股外侧皮神经阻滞镇静效果的临床观察

      【摘要】 目的 比较右美托咪定与咪达唑仑用于超声引导股神经-股外侧皮神经阻滞麻醉的镇静效果,为临床应用提供参考。方法 选择上海交通大学附属第六人民医院南院2019年7月至2020年1月髌骨骨折手术60例,采用随机数表法将患者分为观察组和对照组各30例,观察组于神经阻滞前10 min开始静脉注入右美托咪定(负荷量0.5 μg/kg,10 min注完),对照组于神经阻滞前10 min静脉注入咪达唑仑0.1 mg/kg,比较两组患者入手术室(T0)、神经阻滞前(T1)、手术开始时(T2)、手术开始后 20 min (T3)、手术结束时(T4)的心率(HR)、平均动脉压(MAP)、呼吸(RR)、氧饱和度(SpO2)、镇静评分(Ramsay评分)和术中舒芬太尼的用量及不良反应。结果 观察组和对照组患者分别在T1时的HR [(72.5±5.7)次/min vs (77.1±6.0)次/min]、MAP [(82.1±8.4) mmHg vs (84.8±8.5) mmHg]、Ramsay评分[(2.4±0.5)分 vs (2.7±0.4)分],T2时的HR [(69.6±6.2)次/min vs (79.7±14.8)次/min]、MAP [(78.8±7.9) mmHg vs (83.2±9.0) mmHg]、Ramsay评分[(3.1±0.4)分 vs (3.8±0.41)分],T3时的HR [(67.1±5.9)次/min vs (80.3±7.2)次/min]、MAP[(76.9±8.0) mmHg vs (82.9±7.4) mmHg]、Ramsay评分[(3.5±0.51)分 vs (4.0±0.67)分],T4时的HR [(65.9±5.8)次/min vs(78.5±5.5)次/min]、MAP [(76.3±7.4) mmHg vs (83.4±7.9) mmHg]、Ramsay评分[(3.0±0.61)分 vs (3.7±0.45)分]比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);观察组和对照组患者分别在T1时点的RR [(16.5±1.2)次/minvs (15.7±0.9)次/min]、SpO2 [(97.9±0.8)% vs (96.7±1.3)%],T2时点的RR [(15.4±0.9)次/min vs (12.0±1.3)次/min]、SpO2[(97.8±1.2)% vs (94.0±1.8)%],T3时点的 RR [(15.1±0.7)次/min vs (12.1±1.3)次/min]、SpO2 [(97.0±0.9)% vs (94.5±2.0)%],T4时点的SpO2 [(97.1±0.99)% vs (96.1±1.0)%]比较,观察组明显高于对照组,差异均有统计学意义(P<0.05);观察组患者术中使用舒芬太尼用量为(6.0±2.3) μg,明显少于对照组的(10.8±4.7) μg,差异有统计学意义(P<0.05)。结论 右美托咪定辅助超声引导股神经-股外侧皮神经阻滞麻醉镇静效果良好,而且患者舒适度高,值得临床推广。
