      标题:妊娠晚期孕妇维生素A水平与妊娠期高血压疾病的关系及维生素A对分娩结局的影响

      【摘要】 目的 探讨妊娠晚期孕妇维生素A水平与妊娠期高血压疾病(HDCP)的关系及维生素A对分娩结局的影响。方法 选择汉中市铁路中心医院于2017年1月至2019年6月期间收治的176例HDCP患者为研究对象,将同期150例妊娠健康产妇纳入对照组。根据176例HDCP患者的病情程度分为妊娠期高血压组(n=90)、轻度子痫前期组(n=56)和重度子痫前期组(n=30),比较不同组别受检者的血清维生素A水平。将HDCP患者根据随机数表法分为干预组和未干预组,每组 88例,未干预组患者予常规治疗,干预组则在此基础上给予联合补充维生素A治疗,比较两组患者的分娩结局。结果 血清维生素A水平在对照组、妊娠期高血压组、轻度子痫前期组、重度子痫前期组依次降低,且维生素A缺乏发生率依次升高,差异均有统计学意义(P<0.05);HDCP患者中,干预组患者早产及低体质量儿的发生率分别为 3.41%、2.27%,明显低于未干预组的 7.95%、5.68%,差异均有统计学意义(P<0.05)。结论 妊娠晚期孕妇维生素A水平与妊娠期高血压疾病的发生关系密切,且其水平与疾病的病情程度有关。补充维生素A能降低早产、低体质量儿的发生风险,对改善分娩结局有一定的帮助。

Relationship between vitamin A level and hypertensive disorder complicating pregnancy in late pregnancy andthe effect of vitamin A on delivery outcome.

【Abstract】 Objective To investigate the relationship between vitamin A level and hypertensive disorder com-plicating pregnancy (HDCP) and the effect of vitamin A on delivery outcome. Methods A total of 176 HDCP patientstreated in Hanzhong Railway Central Hospital from January 2017 to June 2019 were selected as the study subjects, and150 pregnant healthy women were included into the control group. According to the degree of HDCP, the patients weredivided into three groups: pregnancy hypertension group (n=90), mild preeclampsia group (n=56), severe preeclampsiagroup (n=30). Patients with HDCP were divided into intervention group and non-intervention group according to randomnumber table, with 88 patients in each group. The patients in the non-intervention group were given routine treatment,while those in the intervention group were treated with vitamin A as supplement based on routine treatment. The deliveryoutcome of the two groups was compared. Results The level of serum vitamin A in the control group, pregnancy hy-pertension group, mild eclampsia group, severe eclampsia group decreased in turn, and the incidence of vitamin A defi-ciency increased in turn, all with statistically significant differences (P<0.05). The incidence of preterm delivery and lowbirth weight in the intervention group (3.41% and 2.27%) was significantly lower than that in the non-intervention group(5.68% after 7.95%), with statistically significant differences (P<0.05). Conclusion The level of vitamin A in late preg-nancy is closely related to the occurrence of HDCP, and the level is related to the degree of disease. Vitamin A supple-mentation reduces the risk of premature and low birth weight infants, and is helpful to improve the delivery outcome.
