标题：腹腔镜阑尾切除术中两种方式处理阑尾根部的对比分析

      【摘要】 目的 研究分析7#丝线三重套扎法和可吸收夹固定法处理阑尾根部在腹腔镜阑尾切除术中的可靠
性和安全性。方法 回顾分析接受腹腔镜阑尾切除术的128例患者的临床资料，根据处理阑尾根部方式不同分
为7#丝线三重套扎法组（A组）和可吸收夹固定组（B组），每组64例，并对感染、粪瘘等术后并发症以及血清前白
蛋白、转铁蛋白等指标进行统计分析。结果 128例患者手术均完全成功，无一例出现并发症，两组患者的血清
前白蛋白、转铁蛋白水平术后1 d均出现下降，但A组患者的血清前白蛋白、转铁蛋白含量在术后3 d的升幅显著
高于B组（P<0.05），A组患者的手术时间显著高于B组，住院时间和医疗费用显著低于B组（P<0.05），两组患者
的下床时间差异无统计学意义。结论 通过两种方式处理阑尾根部在腹腔镜阑尾切除术中的优缺点比较，推断
出两种方法处理阑尾根部都是安全可靠的，而且对机体无刺激，术后无不良反应。但腹腔镜7#丝线三重套扎法
处理阑尾根部术后营养恢复更快，住院时间更短，此技术简单易学，容易在医院普及。

【Abstract】 Objective To study the reliability and safety of using triple loop ligature with 7-0 silk suture
and absorbable clips for treating the root of the appendix during laparoscopic appendectomy. Methods One hundred
and twenty-eight patients undergoing laparoscopic appendectomy were divided into two groups. Group A (n=64) ap-
plied triple loop ligature with 7-0 silk suture to treat the root of the appendix, while group B used absorbable clips.
The indexes of fecal leakage, nutrition, infection, serum prealbumin and transferrin were also detected and ana-
lyzed. Results The operation was performed successfully in all the 128 patients, without complications. On the first
day after operation, the levels of serum prealbumin and transferrin were found to be decreased in the two groups. On
the third day after operation, the increase levels of serum prealbumin and transferrin in group A were significantly larg-
er than those in group B (P<0.05), with significant shorter operation time, shorter length of hospital stay as well as
lower medical costs. The time to get out of bed showed no statistically significant difference between the two groups.
Conclusion The two methods are both safe and reliable for treating the root of the appendix during laparoscopic ap-
pendectomy, with no stimulation and complications. However, triple loop ligature with 7-0 silk suture is superior than
absorbable clips, with sooner recovery of nutrition, shorter length of hospital stay and lower medical costs, which was
easy to popularize in hospitals.
