      标题:耳穴压豆法对慢性阻塞性肺疾病稳定期合并营养不良患者BODE指数的影响

      【摘要】 目的 探讨耳穴压豆法对慢性阻塞性肺疾病(COPD)稳定期合并营养不良患者BODE指数的影响。方法 广州市增城区中医医院呼吸内科2016年7月至2019年6月期间收治的86例COPD稳定期合并营养不良患者以随机数表法分为观察组和对照组,每组43例,对照组给予常规药物、营养支持及氧疗治疗,观察组在对照组基础上加用耳穴压豆治疗,6个月后比较两组患者微型营养评定表(MNA)评分、体质量指数(BMI)、一秒用力呼气容积(FEV1%)、改良英国医学研究会呼吸困难指数(MMRC分级)、6 min步行测试(6MWD)等BODE指标的差异。结果 观察组和对照组患者治疗前的MNA评分[(15.7±4.2)分 vs (15.3±3.9)分]比较差异无统计学意义(P>0.05);经上述治疗后,观察组和对照组MNA评分分别为(21.8±3.8)分及(17.6±3.7)分,均明显高于同组治疗前,且治疗后观察组患者的MNA评分明显高于对照组,差异均具有统计学意义(P<0.05);两组患者治疗前的BMI、FEV1%、mMRC分级、6MWD、BODE指数评分等指标比较差异均无统计学意义(P>0.05);治疗后,两组患者的BMI、FEV1%、mMRC分级、6MWD、BODE指数评分明显优于治疗前,且观察组患者的上述各项指标明显优于对照组,差异均具有统计学意义(P<0.05)。结论 耳穴压豆法应用于慢性阻塞性肺疾病稳定期合并营养不良患者可有效改善患者的BODE各项指标,值得临床推广。

Influence of auricular pressure on body mass index, airflow obstruction, dyspnea, and exercise capacity (BODE)in patients with COPD combined with innutrition.

【Abstract】 Objective To discuss the influence of auricular pressure on BODE in patients with chronic obstruc-tive pulmonary disease (COPD) in stable period combined with innutrition. Methods Eighty-six patients with COPDin stable period and innutrition from Department of Respiratory Medicine, Zengcheng Hospital of Traditional ChineseMedicine during July 2016 and July 2019 were randomly divided into the observation group (n=43) and the controlgroup (n=43) according to random number table. The control group was treated with traditional western medicine, nutri-tional support, and oxygen therapy, while the observation group was given auricular pressure based on treatment of thecontrol group. Then BODE indexes, including the mini nutritional assessment (MNA) score, body mass index (BMI),(forced expiratory volume in one second (FEV1%), modified British medical research council (mMRC), 6-minute walk-ing distance (6MWD), were contrasted between the two groups after 3 months. Results There was no significant differ-ence in MNA score between the two groups before the treatment: (15.7±4.2) vs (15.3±3.9), P>0.05; after the treatment,MNA score in the observation group and the control group were 21.8±3.8 and 17.6±3.7, significantly higher than thosebefore treatment, and the MNA in the observation group was significantly higher than that of the control group (P<0.05).Before treatment, there was no significant difference in BMI, FEV1%, mMRC, 6MWD, BODE between the two groups(P>0.05); after treatment, BMI, FEV1%, mMRC, 6MWD, BODE in the two groups were significantly better than thosebefore treatment (P<0.05), and the levels in the observation group were significantly better than those in the controlgroup (P<0.05). Conclusion Auricular pressure can effectively increase the effective rate and improve the BODE in-dex in the treatment of patients with COPD in stable period and innutrition, which is worthy of promotion.
