      标题:影响颅内动脉瘤破裂的危险因素

      【摘要】 目的 探讨散发性颅内动脉瘤破裂出血的危险因素。方法 回顾性分析海南省人民医院2015年1月至2019年8月收治的205例散发性颅内破裂动脉瘤患者(破裂组)和86例颅内未破裂动脉瘤患者(未破裂组)的临床资料;采用χ2检验及多元Logistic回归对各相关危险因素进行统计学分析。结果 单因素分析结果显示,破裂组与未破裂组患者在性别、吸烟、多发动脉瘤、动脉瘤的大小及动脉瘤的形状等方面比较差异均有统计学意义(P<0.05);Logistic回归分析结果显示,女性、吸烟、多发动脉瘤、5 mm≤直径≤7 mm的动脉瘤、直径≥8 mm的动脉瘤、不规则形状动脉瘤是颅内动脉瘤破裂的独立危险因素(P<0.05)。结论 女性、吸烟、多发动脉瘤、直径≥5 mm的动脉瘤及不规则形状的动脉瘤是颅内动脉瘤破裂的独立危险因素。

Risk factors affecting ruptured intracranial aneurysms.

【Abstract】 Objective To investigate the risk factors of sporadic intracranial aneurysm rupture and hemor-rhage. Methods Retrospective analysis was conducted on the clinical data of 205 patients with sporadic ruptured intra-cranial aneurysm (ruptured group) and 86 patients with unruptured intracranial aneurysm (unruptured group) who admit-ted to Hainan General Hospital from January 2015 to August 2019. The relevant risk factors were statistically analyzedby χ2 test and multivariate logistic regression. Results The univariate analysis showed that the differences between theruptured group and the unruptured group were statistically significant in terms of gender, smoking, multiple aneurysms,aneurysm size, and aneurysm shape (P<0.05). Logistic regression analysis showed that women, smoking, multiple aneu-rysms, aneurysms with a diameter of 5 mm≤d≤7 mm, aneurysms with a diameter≥8 mm, and irregular shaped aneu-rysms were independent risk factors for ruptured intracranial aneurysm (P<0.05). Conclusion Women, smoking, multi-ple aneurysms, aneurysms with diameter ≥5 mm, and irregular shaped aneurysms are independent risk factors for rup-tured intracranial aneurysm.
