标题:膝关节损伤术后早期康复训练对其恢复的促进作用

【摘要】 目的 分析膝关节损伤术后早期康复训练对膝关节功能恢复效果的影响。方法 选择郑州市第七人民医院2015年2月至2017年2月期间进行骨折手术治疗后膝关节功能存在障碍患者共80例,根据随机数表法将患者分为观察组和对照组,每组40例。术后24 h,对照组患者进行连续被动活动(CPM)锻炼,观察组患者则在进行CPM锻炼时配合康复指导训练,训练时长每次30 min,2次/d,持续3周为一个疗程。于疗程结束后,采用Lysholm膝关节评分方法评定膝关节功能,评价指标包括临床疗效、伸屈肌的峰力矩(Peak Torque,PT)以及平均功率(AveragePower,AP)和膝关节的活动度(伸膝最大的角度/屈膝最大的角度)。结果 观察组患者的膝关节功能恢复的总优良率为80.0%,明显高于对照组的47.5%,差异具有统计学意义(P<0.05);观察组患者训练后的伸肌、屈肌时的PT分别为(81.46±18.22) (N·m)/kg、(56.33±8.20) (N·m)/kg,明显大于对照组的(70.84±12.20) (N·m)/kg、(48.25±8.04) (N·m)/kg,AP分别为(35.28±8.34) W、(29.33±5.22) W,明显大于对照组的(26.57±6.13) W、(21.66±4.23) W,差异均具有统计学意义(P<0.05);观察组患者术后 8周及 16周的伸膝最大角度分别为(0.30±0.16)°、(0.56±0.20)°,明显大于对照组的(0.12±0.07)°、(0.23±0.10)°,屈膝最大角度分别为(30.37±5.31)°、(31.44±7.53)°,明显大于对照组的(22.46±4.20)°、(22.36±6.31)°,差异均具有统计学意义(P<0.05)。结论 膝关节损伤术后早期康复训练能有效改善膝关节损伤患者关节的功能,提高肌力的同时扩大膝关节的活动度。

Promoting effect of early rehabilitation training after knee operation on knee function recovery.

【Abstract】 Objective To analyze the effect of early rehabilitation training after knee operation on knee func-tion recovery. Methods A total of 80 patients with knee joint dysfunction after fracture surgery were selected from the7th People’s Hospital of Zhengzhou during February 2015 and February 2017. According to the random number tablemethod, the patients were divided into observation group and control group, with 40 cases in each group. 24 hours afterthe operation, patients in the control group performed continuous passive motion (CPM), and patients in the observationgroup cooperated with rehabilitation guidance training during CPM exercise. The training duration was 30 minutes eachtime, twice a day, lasting for 3 weeks as a course of treatment. At the end of the course, Lysholm knee joint scoring methodwas used to evaluate knee function. The evaluation indexes included clinical efficacy, peak torque (PT), average power(AP), and knee joint activity (maximum knee extension angle/maximum knee flexion angle). Results The total excellentand good rate of knee function recovery was 80.0% in the observation group, which was significantly higher than 47.5% inthe control group (P<0.05). After training, the PTs of the extensors and flexors was respectively (81.46±18.22) n.m/kg and(56.33±8.20) N·m/kg in the observation group, which was significantly higher than corresponding (70.84±12.20) N·m/kgand (48.25±8.04) N·m/kg in the control group; while AP of them was respectively (35.28±8.34) W and (29.33±5.22) W inthe observation group, which was significantly higher than corresponding (26.57±6.13) W and (21.66±4.23) W in thecontrol group; all differences were statistically significant (P<0.05). At 8 and 16 weeks after operation, the maximum ex-tension knee angles was (0.30±0.16) degrees and (0.56±0.20) degrees in the observation group, respectively, which wassignificantly higher than corresponding (0.12±0.07) degrees and (0.23±0.10) degrees in the control group; and the maxi-mum knee flexion angle was (30.37±5.31) degrees and (31.44±7.53) degrees, respectively, which was significantly high-er than corresponding (22.46±4.20) degrees and (22.36±6.31) degrees in the control group; all differences were statisti-cally significant (P<0.05). Conclusion Early rehabilitation training after knee surgery can effectively improve the jointfunction of patients with knee injury, increase muscle strength and expand knee mobility
