      标题:AIDET沟通模式在影像科患者检查前准备中的应用价值

      【摘要】 目的 探讨“问候-自我介绍-过程-解释-感谢”(AIDET)沟通模式对影像科患者冠脉CT成像检查时心理应激反应及行为状态的影响。方法 选择2017年10月至2018年10月于深圳市宝安区松岗街道松岗人民医院影像科80例拟行冠状动脉CT造影成像的老年患者为研究对象,按随机数表法将患者分为观察组和对照组,每组40例。观察组采用AIDET沟通模式进行检查前沟通,对照组则采用传统沟通模式。观察并比较两组患者检查过程中的血压、心率的变化以及皱眉、躲闪、微笑、平静行为的发生情况,比较两组干预前后 90项症状自评量表(SCL-90)评分变化,并比较两组患者的一次检查成功率和出现图像伪影的比例。结果 观察组和对照组患者干预过程中的收缩压 (SBP) [(139.52 ± 7.15) mmHg vs (152.34 ± 10.54) mmHg]、舒张压 (DBP) [(69.25 ± 7.92) mmHg vs(79.51±9.05) mmHg]、心率(HR) [(95.11±8.49)次/min vs (110.52±10.45)次/min]比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);观察组和对照组患者干预后的SCL-90量表中的躯体化[(1.23±0.15)分 vs (1.87±0.35)分]、强迫[(1.40±0.18)分 vs (1.80±0.21)分]、人际关系敏感[(1.46±0.25)分 vs (1.80±0.37)分]、抑郁[(1.10±0.20)分 vs (2.00±0.50)分]、焦虑[(1.13±0.16)分 vs (1.82±0.35)分]等评分比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);观察组患者的平静行为发生率为80.0%,明显高于对照组的57.5%,皱眉、躲闪行为发生率分别为10.0%、5.0%,明显低于对照组和30.0%、37.5%,差异均有统计学意义(P<0.05);观察组患者的一次检查成功率为95.0%,明显高于对照组的80.0%,出现图像伪影比例为5.0%,明显低于对照组的 25.0%,差异均有统计学意义(P<0.05)。结论 AIDET沟通模式可降低老年患者冠脉CT成像检查时心理应激反应,平静渡过检查期,提高CT成像检查效率。

Application value of AIDET communication model in pre-examination preparation of patients in Department ofMedical Imaging.

【Abstract】 Objective To explore the effect of AIDET (Acknowledge, Introduce, Duration, Explanation, ThankYou) communication mode on psychological stress response and behavior status of patients undergoing coronary CT im-aging. Methods A total of 80 elderly patients, who performed CT-coronary angiography in Department of Medical Im-aging, Songgang People's Hospital of Baoan District of Shenzhen from October 2017 to October 2018, were selected di-vided into the observation group and control group according to random number table, with 40 cases in each group. Theobservation group used AIDET communication mode to communicate before examination, and the control group usedtraditional communication mode. The changes of blood pressure, heart rate, frowning, dodging, smiling and quiet behav-ior were observed during the examination in the two groups. The changes of symptom check-list-90 (SCL-90) scores be-fore and after the intervention were observed. The success rate of one-time examination between the two groups wascounted, and the difference of artifact ratio was found. Results During the intervention, the systolic blood pressure(SBP), diastolic blood pressure (DBP), heart rate (HR) in the observation group were (139.52±7.15) mmHg, (69.25±7.92) mmHg, (95.11±8.49) times/min, which were significantly lower than corresponding (152.34±10.54) mmHg, (79.51±9.05) mmHg, (110.52±10.45) times/min in the control group (All P<0.05). After the intervention, the somatization, com-pulsion, interpersonal sensitivity, depression, anxiety in the SCL-90 scale in the observation group were 1.23±0.15, 1.40±0.18, 1.46±0.25, 1.10±0.20, 1.13±0.16, respectively, which were significantly lower than corresponding 1.87±0.35, 1.80±0.21, 1.80±0.37, 2.00±0.50, 1.82±0.35 in the control group (all P<0.05). The incidence of quiet behavior in the observationgroup was 80.0%, which was significantly higher than 57.5% in the control group (P<0.05); the incidence of frowning anddodging behavior were 10.0%, 5.0% respectively, which were significantly lower than 30.0%, 37.5% in the control group(P<0.05); the success rate of one examination in the observation group was 95.0%, which was significantly higher than80.0% in the control group (P<0.05); the proportion of image artifacts was 5.0%, which was significantly lower than 25.0%in the control group (P<0.05). Conclusion AIDET communication mode can reduce the psychological stress response ofelderly patients during coronary CT imaging, quietly pass the examination period, and improve the efficiency of CT imaging.
