      【摘要】 目的 分析血清单项乙型肝炎病毒核心抗体阳性的可能原因并探讨其临床意义。方法 收集酶
联免疫吸附竞争一步法检测的500例单项乙型肝炎病毒核心抗体阳性血清,用微粒子酶免疫法进行验证。分析
单项乙型肝炎病毒核心抗体阳性血清的丙型肝炎病毒抗体、人类免疫缺陷病毒抗体及乙型肝炎病毒DNA。
结果 在酶联免疫吸附竞争一步法检测的单项乙型肝炎病毒核心抗体阳性血清中,用微粒子酶免疫法进行检测
显示乙型肝炎病毒核心抗体阳性率仅为67.2%。单项乙型肝炎病毒核心抗体阳性标本中丙型肝炎病毒抗体阳性
率为5.36%;可检测到乙型肝炎病毒DNA为3.87%,DNA拷贝数为(159±64) copies/ml;未发现人类免疫缺陷病毒
抗体阳性。结论 酶联免疫吸附竞争一步法检测乙型肝炎病毒核心抗体存在一些假阳性结果。隐性乙型肝炎
病毒感染、丙型肝炎病毒感染是血清单项乙型肝炎病毒核心抗体阳性的原因。

Causes and clinical significance of serological pattern of anti-HBc alone.

【Abstract】 Objective To investigate the factors causing serological pattern of anti-HBc alone, and to ex-
plore the clinical significance of anti-HBc alone in population. Methods Serum of anti-HBc alone was collected
from individuals using one step enzyme-linked immunosorbent assay. Anti-HBc in serum was also measured by mic-
roparticle enzyme immunoassays. Anti-HCV and anti-HIV were tested with enzyme-linked immunosorbent assay.
HBV DNA in serum was measured with real-time quantitative polymerase chain reaction. Results Microparticle en-
zyme immunoassay showed that positive rate of serum anti-HBc alone was only 67.2% for positive sample tested by one
step enzyme-linked immunosorbent assay. The HCV-infection rate was 5.36% for all anti-HBc alone individuals. HBV
DNA was presented in 3.87% individuals with serum anti-HBc alone, copies of HBV DNA were (159±64) copies/ml for
these individuals. HIV-infection was not found among anti-HBc alone individuals. Conclusion A false positive out-
come may arise for anti-HBc alone with one step enzyme-linked immunosorbent assay. Occult HBV infection and
HCV-infection are factors causing serological positive pattern of anti-HBc alone.
