      【摘要】 目的 探讨CT动态增强扫描在甲状腺结节性病变良恶性鉴别诊断中的应用价值。方法 选择我
院收治的甲状腺结节性病变患者85例,其中良性结节55例,恶性结节30例,均经手术病理证实。所有病例均进
行动态增强扫描,比较良恶性结节的动态增强CT特征及其曲线变化的差异。结果 30例甲状腺恶性结节中25
例表现为均匀性强化,5例不均匀强化;55例良性结节中,11例呈均匀强化,26例不均匀强化,18例无强化。良性
结节与恶性结节间三种强化方式的显示率差异有统计学意义(P<0.05)。良性结节与恶性结节的TDC曲线表现
差异有统计学意义(P<0.05)。甲状腺恶性结节的强化峰值和强化增加值均显著低于良性结节,差异具有统计学
意义(P<0.05)。结论 CT动态增强扫描对甲状腺结节性病变良恶性鉴别诊断具有一定的应用价值,全面分析结
节的强化方式、强化幅度以及TCD曲线特性能进一步提高诊断的准确性。

Value of the contrast-enhanced multi-slice spiral CT in the differential diagnosis of benign and malignant
thyroid nodule.

【Abstract】 Objective To study the value of the contrast-enhanced multi-slice spiral CT in the differential di-
agnosis of benign and malignant thyroid nodule. Methods CT imaging of 85 patients with thyroid nodules diag-
nosed by operation and histopathological examination were analyzed retrospectively, including 30 cases of malignant
thyroid nodule and 55 cases of benign thyroid nodule. The differences between benign and malignant thyroid nodules
were compared. Results Among the 30 cases of malignant thyroid nodule, 25 were even improved, 5 were uneven
improved. Among the 55 cases of benign thyroid nodule, 11 were even improved, 26 were uneven improved, and 18
were not improved. There were statistically significant different in enhanced mode between benign and malignant
thyroid nodule (P<0.05). There were statistically significant difference in TDC curve between benign and malignant
thyroid nodule (P<0.05). The peak of enhancement and the added of strengthen value in malignant thyroid nodule
were lower than those in benign thyroid nodule (P<0.05). Conclusion The contrast-enhanced multi-slice spiral CT
are important in the differential diagnosis of benign and malignant thyroid nodule. It can improve the accuracy of diag-
nosis by analyzing enhanced mode, enhanced range and TCD curve.
