标题:全程护理对睡眠呼吸暂停低通气综合征射频温控消融治疗术后患儿恢复及睡眠状况的影响

【摘要】 目的 探讨全程护理对睡眠呼吸暂停低通气综合征(OSAHS)射频温控消融治疗术后患儿恢复及睡眠状况的影响,为临床应用提供指导。方法 选取2016年9月至2018年9月在深圳市儿童医院耳鼻喉科接受治疗的 60例OSAHS患儿作为研究对象,按照随机数表法分为观察组和对照组各 30例。所有患儿进行射频温控消融治疗术后,对照组患儿采用常规护理,观察组患儿采用全程护理。比较两组患儿治疗前后的嗜睡评分量表(Epworth)评分、生活质量指数量表(Calgary)评分及手术相关指标;比较两组患儿的疗效、术后并发症以及随访情况。结果 治疗后,观察组患儿的Epworth评分及呼吸暂停低通气指数分别为(3.61±0.85)分和17.38±8.20,明显低于对照组的(4.58±1.23)分和22.53±11.35,Calgary评分及动脉血氧饱和度分别为(5.21±1.20)分和86.58±8.57,明显高于对照组的(4.50±0.84)分和82.29±6.58,差异均有统计学意义(P<0.05);观察组患儿的治疗有效率为100.00%,明显高于对照组的86.67%,差异有统计学意义(P<0.05);观察组患儿中出现继发性出血者1例,对照组中出现2例。随访期间,两组患儿均未出现进食反流及闭塞性鼻音症。结论 全程护理对OSAHS射频温控消融治疗术后患儿恢复及睡眠状况具有积极影响,其不仅有助于患儿恢复,缓解嗜睡,提高血氧饱和度,还可帮助患儿通气,提高患儿的生活质量和治疗有效率。

Effect of whole-course nursing on the recovery and sleep status of postoperative children with obstructivesleep-apnea-hypopnea syndrome by radiofrequency ablation and ablation.

【Abstract】 Objective To explore the effect of full-course nursing on the recovery and sleep status of postopera-tive children with obstructive sleep apnea hypopnea syndrome (OSAHS) by radiofrequency ablation, and provide guid-ance for clinical application. Methods Sixty children with OSAHS, who admitted to Department of Otorhinolaryngolo-gy, Shenzhen Children's Hospital from September 2016 to September 2018 were selected and divided into the observa-tion group and the control group according to random number table method, with 30 cases in each group. All patients un-derwent radiofrequency ablation and ablation. The control group received routine care and the observation group re-ceived full-course care. The Epworth score, quality of life index (Calgary) score, and surgical related indicators werecompared between the two groups. The efficacy, postoperative complications, and follow-up of the two groups were alsocompared. Results After treatment, the Epworth score and apnea hypopnea index in the observation group were 3.61±0.85 and 17.38±8.20, respectively, which were significantly lower than corresponding 4.58±1.23 and 22.53±11.35 in thecontrol group (P<0.05); the Calgary score and arterial oxygen saturation were 5.21±1.20 and 86.58±8.57, respectively,which were significantly higher than corresponding 4.50±0.84 and 82.29±6.58 in the control group (P<0.05). The ef-fective rate of treatment in the observation group (100.00% ) was significantly higher than that in the control group(86.67%), P<0.05. One patient with secondary bleeding occurred in the observation group, and two patients in the con-trol group. During the follow-up period, no reflux or occlusive nasal rhythm occurred in either group. Conclusion Thewhole course of care has a positive effect on the recovery and sleep status of postoperative children with obstructivesleep apnea hypopnea syndrome. It not only helps the child's postoperative recovery, thereby reducing drowsiness andincreasing oxygen saturation, but also help children ventilate, so as to improve the quality of life and treatment effi-ciency of children.
