      标题:纤维支气管镜吸痰灌洗治疗呼吸重症监护室肺部感染临床分析

      【摘要】 目的 分析纤维支气管镜吸痰灌洗治疗呼吸重症监护室肺部感染的临床效果。方法 于2015年6月至2017年6月选择在长沙市第一医院(南华大学附属长沙医院)呼吸重症监护室住院治疗的80例肺部感染患者作为研究对象。按照随机数表法将患者分为观察组和常规组,每组40例。常规组采取常规吸氧、抗感染、机械通气以及补液等治疗措施,观察组在常规组基础上采取纤维支气管镜吸痰灌洗治疗。统计并比较两组患者的治疗效果、机械通气时间、抗菌药物使用时间和 ICU住院时间。结果 观察组患者的治疗总有效率为90.0%,明显高于常规组的80.0%,差异有统计学意义(P<0.05);观察组患者的机械通气时间、抗菌药物使用时间和 ICU住院时间分别为(8.7±2.1) d、(8.0±1.6) d和(14.7±3.8) d,均明显短于常规组的(14.2±3.3) d、(13.4±3.1) d和(23.8±3.5) d,差异均有统计学意义(P<0.05)。结论 纤维支气管镜吸痰灌洗治疗呼吸重症监护室肺部感染的临床效果突出,其能够有效改善患者气道分泌物和临床症状,缩短机械通气时间,值得推广应用。

【Abstract】 Objective To analyze the clinical effect of fiberoptic bronchoscopic sputum aspiration and bron-choalveolar lavage in the treatment of pulmonary infection in respiratory intensive care unit. Methods From June 2015to June 2017, 80 hospitalized patients in the Respiratory Intensive Care Unit, the First Hospital of Changsha (ChangshaHospital Affiliated to University of South China) were selected and randomly divided into experimental group and con-ventional group, with 40 cases in each group. The conventional group received routine treatments of oxygen inhalation,anti-infection, mechanical ventilation and fluid replacement. The experimental group received fiberoptic bronchoscopicsputum aspiration and bronchoalveolar lavage on the basis of conventional group. The treatment effect, mechanical venti-lation time, time of using antibacterial drug, and ICU hospitalization time were statistically analyzed and compared be-tween the two groups. Results The total effective rate of the patients in the experimental group (90.0%) was significant-ly better than that in the conventional group (80.0%), P<0.05. The mechanical ventilation, time of using antibacterialdrug, and ICU hospitalization time in the experimental group were (8.7±2.1) d, (8.0±1.6) d, and (14.7±3.8) d, respective-ly, which were significantly shorter than (14.2±3.3) d, (13.4±3.1) d, and (23.8±3.5) d in the conventional group (P<0.05).Conclusion Fiberoptic bronchoscopic sputum aspiration and bronchoalveolar lavage had clinical effect in the treat-ment of pulmonary infection in the respiratory intensive care unit. It can effectively improve the patient's airway secre-tion and the clinical symptoms and shorten the mechanical ventilation time, which is worth popularizing.
