标题：全髋关节置换术治疗老年髋关节发育不良继发骨关节炎疗效观察

      【摘要】 目的 探讨全髋关节置换术(THA)治疗老年髋关节发育不良(DDH)继发骨关节炎(OA)患者的临床效果。方法 选取宝鸡市人民医院骨一科 2013年 2月至 2017年 5月期间收治的老年DDH继发OA患者 60例给予THA治疗，比较治疗前后患者的Harris评分、关节活动度及并发症情况。结果 治疗后患者的CroweⅠ级Harris评分为(97.14±4.26)分，明显高于治疗前的(47.18±4.22)分；CroweⅡ级Harris评分为(95.18±3.26)分，明显高于治疗前的(42.12±3.23)分；CroweⅢ级Harris评分为(89.21±3.88)分，明显高于治疗前的(41.11±4.12)分；CroweⅣ级Harris评分为(77.15±6.21)分，明显高于治疗前的(42.13±3.52)分，差异均具有统计学意义(P<0.05)；治疗后患者的髋关节屈曲活动度为(127.91±12.15)°，明显高于治疗前的(96.31±22.12)°；外展活动度为(40.25±4.11)°，明显高于治疗前的(18.81±12.15)°；内收活动度为(26.31±22.12)°，明显高于治疗前的(15.21±12.77)°；外旋活动度为(46.34±14.42)°，明显高于治疗前的(23.31±12.55)°；内旋活动度为(26.21±4.15)°，明显高于治疗前的(4.18±4.21)°，差异均具有统计学意义(P<0.05)；术后所有患者均未出现明显并发症。结论 THA术可显著改善老年DDH继发OA患者髋关节功能，提高患肢活动能力，且具有较高的安全性。

Clinical effect of total hip arthroplasty in elderly patients with osteoarthritis secondary to developmentaldysplasia of the hip.

【Abstract】 Objective To discuss the clinical effect of total hip arthroplasty (THA) on elderly patients with os-teoarthritis (OA) secondary to developmental dysplasia of the hip (DDH). Methods A total of 60 elderly patients withosteoarthritis secondary to developmental dysplasia of the hip, who admitted to the First Department of Orthopedics ofBaoji People's Hospital from Feb. 2013 to May 2017, were treated with THA. Harris score, joint activity and complica-tions before and after treatment were compared. Results After the treatment, the Harris scores Crowe Ⅰ, Crowe Ⅱ,Crowe Ⅲ, Crowe Ⅳ of the patients were (97.14±4.26), (95.18±3.26), (89.21±3.88), (77.15±6.21), respectively, whichwere significantly higher than corresponding (47.18±4.22), (42.12±3.23), (41.11±4.12), (42.13±3.52) before the treat-ment (all, P<0.05); the flexion mobility of hip joint, abduction activity, external rotation activity, internal rotation activityof the patients were (127.91±12.15)°, (40.25±4.11)°, (26.31±22.12)°, (46.34±14.42)°, (26.21±4.15)°, respectively, whichwere significantly higher than corresponding (96.31±22.12)°, (18.81±12.15)°, (15.21±12.77)°, (23.31±12.55)°, (4.18±4.21)° before the treatment (all, P<0.05). Conclusion THA can significantly improve the function of the hip joint inthe elderly patients with osteoarthritis secondary to developmental dysplasia of the hip, and improve the mobility of theaffected limbs, which also has high security.
