      标题:少见部位神经鞘瘤的CT和MRI表现

      【摘要】 目的 了解少见部位神经鞘瘤的CT和MRI征象,以提高该疾病临床诊断率。方法 回顾性分析2011年11月至2018年3月吉林大学中日联谊医院收治的90例经病理学证实的少见部位神经鞘瘤影像资料。病变发生在颈部13例,胸部10例,腹部20例,四肢8例,椎管内35例,臀部2例,臂丛2例。32例行CT检查,其中16例行增强扫描;58例行MRI检查,其中32例行增强扫描。结果 90例病灶均为良性,大多呈结节状、长条状,其中8例病灶平扫密度均匀,3例可见钙化成分,所示病灶均界清,增强扫描呈轻中度渐进性强化。MRI平扫T1WI大多呈等或稍低信号,T2WI呈稍高信号,部分病灶内信号混杂,增强扫描不均匀强化,其中仅11例信号均匀,7例可见典型“同心圆征”或“靶征”。结论 神经鞘瘤可发生在身体任何部位,少见部位神经鞘瘤的影像学表现仍具有特征性,但在鉴别诊断需要尤为重视,确诊需依靠病理学。

CT and MRI signs of rare schwannoma.

【Abstract】 Objective To investigate the CT and MRI findings of rare schwannoma to improve the clinical di-agnosis rate of the disease. Methods Images of 90 cases of pathologically confirmed rare schwannoma from Novem-ber 2011 to March 2018 at China-Japan Union Hospital of Jilin University were retrospectively analyzed. The lesions oc-curred in 13 cases in the neck, 10 in the chest, 20 in the abdomen, 8 in the limbs, 35 in the spinal canal, 2 in the hips, and2 in the brachial plexus. 32 cases underwent CT examination, of which 16 underwent enhanced scan; 58 cases underwentMRI, of which 32 underwent enhanced scan. Results The 90 cases were all benign, most of which showed nodular andlong strips. CT scans displayed a homogeneous isodense lesion in 8 cases and calcification in 3 cases, with clear bound-ary, and their enhanced scan showed mild to moderate progressive enhancement. On MRI, the parenchyma of schwanno-ma appeared equal or slightly lower T1 signal and slightly higher T2 signal. In some lesions, the signals were mixed, andthe enhanced scan was unevenly enhanced. Among them, only 11 cases displayed a homogeneous signal, and 7 casesshowed typical“concentric circle sign”or“target sign”. Conclusion Schwannoma can be found in any site of thebody. The imaging signs of schwannomas in rare sites are still characteristic, but they require special attention in differen-tial diagnosis. The diagnosis depends on pathology.
